    Our goal is to provide the best sports photos at Lafayette High School.

    Before continuing with this form, there are a few key items to review.

    1. We need a different form for each athlete unless you are having them photographed together and purchasing their package together.  After completing this form, you will have the option to start again with the next athlete.

    2. You can choose from Build Your Own, Package 1, or Package 2 for multiple athletes.

    3. Packages are 1 single pose for the entire package.  If you prefer to mix and match, use Build Your Own package options or Add-On's.

    4. To expand each package option or to make a purchase, please check the box to the left of the package.
