Liability Waiver/ Photo Release: I/ my children/ immediate family have chosen to participate in Djoli Kelen Inc. Birthday Weekend Activities. I/ We understand the nature of the activities and that Djoli Kelen Inc and/or any individual associated is not responsible for any physical injury caused by participating in such activities. I agree, on be half of myself, minor participants, and immediate family that I understand and agree to release, hold harmless, and discharge Djoli Kelen Inc. and/or any associated individual from all claims, costs, liabilities, expenses or judgments, including attorneys’ fees and court costs for any occurrences in connection with participation. I/ we assume all risks in connection with any instruction and further release Djoli Kelen Inc. and its founders, board, administration, contractors, and employees from liability for any injury sustained while enrolled in any dance/drum instruction program or events, including all risks reasonably connected with such activity whether foreseen or unforeseen. I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for my participation in these activities and that Djoli Kelen Inc. does not provide accident or health insurance for those participating in its instruction, activities or programs. I/ we authorize and agree that Djoli Kelen Inc. may take and use photographs, videos or likenesses of myself as needed for documentation, advertisement and/or public relations projects and that I have no rights to the same and will not be compensated for the same. My below acknowledgement confirms that I/ we agree to all terms and conditions listed above and that I am of lawful age and competent to acknowledge this affirmation.
