Emergency Authorization
In the case of an emergency, I (parent or guardian), the undersigned, do hereby authorize Yeshiva Day School of Las Vegas and Camp Reena, its Officers or any of its Administrative staff to be our general agent to consent to any emergency care deemed advisable and to be rendered through general or specific supervision of any licensed physician or surgeon. It is understood that this authorization is given in advance of any specific need for treatment but is given to provide authority to the aforesaid agent to give specific consent to any and all emergency treatment or hospital care which the physician in the exercise of his best judgment may deem advisable. I hereby authorize and empower such individuals to transport, sign admission slips, consent forms, financial responsibility forms, share confidential medical information and any other action necessary for the admission of our child(ren) to a hospital or for treatment by a physician or surgeon. I hereby release, and forever discharge, and agree to indemnify and hold harmless the Yeshiva Day School of Las Vegas and Camp Reena, its officers, directors, employees and agents from all loss, cost, expense, and liability by reason of any action taken hereunder.
Transportation Waiver
I, the undersigned parent/guardian, understand and accept the inherent risks of transporting my child(ren) by bus or car to and from Camp Reena. I release, discharge, and hold harmless Camp Reena, its directors, officers, employees, agents, and volunteers from any and all liability for any loss, damage, or injury, including death, that may occur during such transportation. I also agree to indemnify Camp Reena against any claims arising from this transportation. In case of emergency, I authorize Camp Reena to obtain necessary medical treatment for my child(ren)(ren) and accept responsibility for any associated costs.
Water Activities Waiver
I acknowledge and agree to the following:
I understand that my child(ren) may participate in water activities, including swimming trips, during their time at Camp Reena. I understand that these activities involve inherent risks, including, but not limited to, drowning, injury from water-related accidents, and other unforeseen incidents. I hereby release, waive, discharge, and covenant not to sue Camp Reena, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child(ren) while participating in any water activities.
Field Trip Waiver
I acknowledge and agree to the following:
I understand that my child(ren) may participate in field trips organized by Camp Reena. I understand that field trips involve inherent risks, including, but not limited to, travel-related accidents, injury, illness, and other unforeseen incidents. I hereby release, waive, discharge, and covenant not to sue Camp Reena, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of any loss, damage, or injury, including death, that may be sustained by my child(ren) while participating in any field trips.
Camp Reena Liability Waiver and Release Agreement
I agree that this Waiver and Release of Liability shall apply to each day my child(ren) is at Camp Reena regardless of the date that this form is submitted. I agree I will assume the risk and full responsibility for any and all injuries, losses, or damages, that might occur to my child(ren) or other family members while on the premises of the camp or participating in any off-site camp program or activity including trips; and to the maximum extent of law, I agree to waive and release any and all claims, suits, or related causes of action against Camp Reena, Yeshiva Day School of Las Vegas, their owners, officers, employees or agents for injury, loss, death, costs or other damages to me, my heirs, or assigns, or third party claims, suits or related causes of action asserted against the camp arising my conduct and/or my family's conduct while participating in the camp's programs or activities. I further agree to release, indemnify, defend and hold Camp Reena and Yeshiva Day School of Las Vegas harmless from any liability whatsoever for future claims presented by my child(ren) for any injuries, losses or damages.
Allergy Food Restriction Consent and Waiver
Parents agree not to send any food items to camp containing allergens identified by other campers' allergy information. By enrolling their child(ren), parents agree to adhere to the allergy policy and acknowledge the inherent risks associated with allergic reactions. Parents waive any liability against Camp Reena in the case of harm resulting from an allergic reaction during camp activities.
Consent for Inclusion in Parent Roster
I agree to have my name, telephone number, and address included on the parent roster which will be made available upon request to any parent whose child(ren) is enrolled in camp.
Photo Release Agreement
I grant permission to Camp Reena, its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child(ren), myself, and members of my family while at Camp Reena for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release Camp Reena, and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use the statements of my child(ren), myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, my child(ren)’s rights, and my family’s rights to any and all compensation stemming from the use of these materials.
Parental Consent for Camp Activities
I give my child(ren) permission to participate in all camp activities unless otherwise provided herein.

Dropoff/Pickup Policy
Dropoff:
Begins at 9:00am until 9:15am
Pickup:
Monday-Thursday: 2:50pm, ends at 3:00pm
Friday: 1:50pm, ends at 2:00pm
Any pickup after 3 PM M-Th or 2 PM on Friday will result in a fee prorated at $50 per 15 min.
Payment Terms:
Application Fee:
- A $100 application fee will be charged when you submit this form.
- If your child is not accepted, this fee will be refunded.
- If your child is accepted, the fee will be applied to the total camp cost.
Deposit & Payment Schedule:
Once accepted, you'll receive a registration form. At that time, you will choose and pay the first installment of your payment plan.
Additional Terms:
- No monetary credit will be given for any days in which a camper is absent.
- Dresscode that applies throughout the year in YDLV are in effect in camp as well.
- Girls leaving K-8 must wear skirts that come below the knee. Girls leaving grades 5-8 must wear tops that cover the collarbone and elbows. All children's swimwear must cover their stomach.
- Food and kashrus policies remain consistent with YDLV as well: see excerpt below:
- No electronics may be brought to camp
- Camp Reena will do its best to return all labeled items. Any items left in camp and not claimed by August 18th will become hefker and be disposed of as camp sees fit.
