PROM

This is a high school themed semi-formal dance. Have fun getting dressed up for dinner, dancing, music, friends, games, and more!

Open to 9th-12th grade students

DATE: Saturday May 9th

TIME: 5:30pm-9:30pm

LOCATION: San Clemente Private Estate - address will be emailed to all registered attendees the week of Prom

Ticket price includes:

Dinner, Dessert, Drinks, Music, Dancing, Games, Entertainment

SUPERVISON:

This is a fully chaperoned event. All chaperones are Live Scanned and Backgound Checked. Only students are allowed to attend the event, and parents must select their pick up preference below for after the event. There is no re-entry.

DRESS CODE: Colorful Theatrical Semi-Formal

Ladies - Dresses must be at least fingertip length (dress must extend past the fingertips when arms are relaxed at the side)
Men - Collared Shirts, Dress Pants, Suits, Ties
No large purses or backpacks will be permitted. A large purse or backpack is defined as larger than a shoe box.
Students must not wear apparel that reveals visible cleavage, any part of the stomach or undergarments.
Students must not wear apparel with any cut-outs.
Themed attire is encouraged but not required: colorful theatrical attire, top hats, tailcoat jackets, creative hairstyles and accessories

PICTURES:

Parents, other than our pre-approved chaperones, will not be permitted inside the venue past the door entry drop off. There are no photo areas for parents outside the venue, however students can always take their own pictures in the venue.

Medical Release and Authorization As Parent and/or Guardian of the named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, or x-ray examination for the named child. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to The Red Apple Academy and its affiliates including Directors and Staff to provide the needed emergency treatment prior to the child’s admission to the medical facility. Release authorized on the date of the event. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Release of Liability I understand that there are certain dangers, hazards, and risks inherent in educational activities, and have signed this document in full recognition and appreciation of the dangers of these activities, which dangers include, but are not limited to, physical injuries (minimal, serious or catastrophic) and/or property loss or damage. In consideration of, and in return for, the service, facilities, and other education provided to students by the personnel of RAA, I, along with any of my assignees, heirs, and legal representatives release The Red Apple Project and Red Apple Academy, and all personnel, employees, tutors, teachers, directors, and officers of these entities, from any and all liability, claims and actions that may arise from injury, harm or death to my student and from loss or damage to my student’s property in connection with these activities. I understand that this release covers liability, claims and actions caused entirely or in part by any acts or failure to act by these individuals and entities, including but not limited to negligence, mistake, or failure to supervise by these individuals or entities or any of their personnel, directors, employees, tutors, or teachers.
