Waiver and Release of Liability:
Release of Liability: I hereby agree to allow my child(ren), and/or myself to voluntarily participate in said named Minnesota Flyers Track and Field activity and authorize Minnesota Flyers Track and Field, Inc., to process my payment electronically.
I confirm that I and the registrant have read, understand, and agree to the Athlete Code of Conduct, Parent Code of Conduct, Volunteer Policy, Pole Vault Policy (for age appropriate registrants), Travel Policy, Relay Policy, and Age Verification Request as available for viewing and download on our website policies page HERE (minnesotaflyers.com/policies) and via the links above. I further confirm I will promptly review the parent orientation presentation via recording or PDF, and agree that registrant will receive their season gear once attendance of the presentation is confirmed.
I understand that under the guidance and supervision of only Minnesota Flyers coaches, workouts from time to time for athletes in the Intermediate and Competitive levels may take place in the neighborhoods surrounding the practice facilities for the purposes of quality training and preservation of physical health. The registrant and I also agree to abide by the rules of Minnesota Flyers Track and Field, USATF, AAU, ISD #270, ISD #281, ISD #283, the University of Minnesota, the Cities of Hopkins, Minnetonka, Plymouth, Minneapolis, and St. Louis Park and their affiliated organizations and/or sponsors.
Assumption of Risk: I hereby, for myself and my heirs, release, discharge, and waive any and all rights and claims for damages, and/or otherwise indemnify Minnesota Flyers Track and Field Inc., its above named affiliated organizations and sponsors, their employees and associated personnel, including any facilities and equipment utilized for the program, as a result of the registrant’s participation in the program and/or for any and all injuries from whatever cause suffered by the above participation in Minnesota Flyers Track and Field activities.
In the event of sudden illness, accident, injury, or other emergency which may occur while said participant is engaged in an activity supervised by Minnesota Flyers Track and Field, the staff has my permission and consent to use their judgment as deemed necessary under the circumstances by Minnesota Flyers Staff with regard to medical treatment including transportation and care by a duly licensed Doctor of Medicine or Doctor of Dentistry which I hereby authorize until I can be contacted, and I release and hold harmless any member of the Minnesota Flyers Track and Field organization and the aforementioned organizations, schools, and cities showing under "Release of Liability," and their affiliated organizations and/or sponsors.
Pictures and videos taken of participants enjoying Minnesota Flyers Track and Field activities are often used for program promotion and teaching. I hereby grant permission to use media and quotes of registrant(s) for these purposes.
Behavior Policy: I recognize that disruptive and/or offensive behavior can affect the safety and quality of the learning environment for all involved. Should registrant display disruptive behavior, coaches and staff have my permission to remove registrant from the vicinity as needed. Should registrant display disruptive and/or offensive behavior three times during the season, I agree that registrant will be removed from the program. I further understand and agree that COACHES and STAFF will have the ultimate say in any concerns or judgement calls regarding this policy and may act upon those concerns to the best of their ability. I understand this policy is in effect to preserve the safety and quality of experience for all participants involved, and agree to abide by the Minnesota Flyers Track and Field behavioral policy as indicated above. There will be no refund provided for any loss of time due to session removal or for an individual removed during the season from the program due to violation of the Behavior Policy.
Right of Refusal: The Minnesota Flyers Track and Field Organization and it’s representatives may, at it’s/their sole discretion, terminate this agreement without advance notice and refuse any registrant’s registration and/or participation with the Minnesota Flyers Track and Field Organization for any of the following reasons not limited to: a) the registrant’s past or current conduct is deemed to be in violation of the Athlete Code of Conduct, b) the registrant has received a Notice of Violation of the Athlete Code of Conduct, c) the registrant’s parent/guardian/family/designated signee is deemed to be in violation of the Athlete and/or Parent Codes of Conduct, and/or, d) the registrant’s parent/guardian/family/designated signee has received said notice of violation of the Athlete and/or Parent Codes of Conduct.
Digital Rights Agreement: Any materials provided either digitally or virtually are the intellectual property of Minnesota Flyers coaches and staff. Material is to be used for training purposes of said registrant only; any other use is prohibited and content may not be copied, shared, published or in any way reproduced.
Participation Policy and Payment Disclosure: Registration will be complete only when both form and payment are submitted. Non-paid registrants will not be able to participate until such time as payment is made. Due to high demand, we are not able to "hold" spots. I understand that Minnesota Flyers Track and Field (MNF) registration fees include important per athlete liability and use costs that need to be paid to MNF providers and affiliates prior to registrant participation, so only paid registrants are able to participate. I therefore acknowledge that my/my child's registration fees must be paid in full prior to the first day of the registered training session and/or access digital training content in order for my child(ren), and/or myself to participate. Should such registration fee(s) remain unpaid prior to the first day of participation, I understand that registrant will not be able to participate and may forfeit enrollment in the program in the event registration is at capacity and/or a waiting list is in effect.
Cancellation Policy: I understand I may cancel by noon on Friday of the first week of training and I must contact Minnesota Flyers Track and Field in writing to admin@minnesotaflyers.com before 6:00pm January 4, 2026 to request to cancel; I further understand any refund will be less costs incurred and said costs must be deducted from the registration fee refund and understand that after such time all fees will be fully processed so agree that there will be no refunds issued for any reason. Minnesota Flyers Track and Field reserves the right to cancel individual sessions due to weather, facility closure, or any other necessary emergencies. Cancelled sessions are non-refundable however, every effort will be made to offer a make-up session.
Electronic Signature Agreement: By continuing below, you agree your electronic signature is the legal equivalent of your manual signature on this agreement and are signing your acceptance and agreement to this waiver and release's terms and conditions. You also agree that no certification authority or other third party verification is necessary to validate your E-signature and the lack of such certification or third party verification will not invalidate this document or affect the enforceability of your E-signature.
By clicking the "Acknowledgement" box below and entering a signature in the electronic signature box, you confirm you are the parent or legal guardian of the registrant as entered in the "Parent/Guardian" portion of this registration form, that you give your consent for registrant to participate with Minnesota Flyers Track and Field, and that you are aware of all terms and conditions in this document in it's entirety.
